Credentialing is the step that lets us verify your qualifications and get you cleared to see veterans. To keep things moving, we ask that you return all required documents within one week of signing your contract. Because our partner agencies adjust their scheduling and provider needs frequently, we cannot guarantee continued need in your location if credentialing is delayed.
Your credentialing checklist
Gather the following items. A few have specific requirements, so read each note carefully before you submit.
- Malpractice insurance certificate that lists your name as the provider.
- Current CV with both start and end month and year for your education and work history.
- Work gap history, required if any gap longer than 30 days falls after your highest level of education.
- Proof of education. If the name on it differs from your current legal name, include legal documentation such as a marriage certificate or divorce decree.
- Color copy of a government-issued photo ID (see accepted types below).
- Proof of citizenship (see accepted types below).
- Copy of your active, unrestricted professional license in your specialty.
- Copy of your diploma or degree certificate.

Choose one of the following:
- U.S. birth certificate together with a photo ID
- Valid U.S. passport
- Certificate of citizenship

- If the name differs from your current legal name, include legal documentation
Tips to avoid delays
- Double-check that your name appears consistently across documents. Mismatched names are the most common cause of delays.
- Make sure your license is current and unrestricted before you submit it.
- Scan documents in color and confirm they are clear and fully readable.
- Include month and year for every entry on your CV, not just the year.
What if my name changed?
Include legal documentation that connects your former and current names, such as a marriage certificate or divorce decree. Add it anywhere a name mismatch could come up, including proof of education and proof of citizenship.
Do I need work gap history if I have no gaps?
Only if a gap longer than 30 days falls after your highest level of education. If that does not apply to you, you can skip it.
